• Did you recently move and need to change your address? If you bill for yourself (meaning you don’t work for an agency), you will need to send Provider Connections a service matrix with your updated information (this can be faxed to: 1-309-298-3066) to change your address with the CBO. If you work for an agency, you will only need to send an email letting us know you would like your address updated and what you would like it updated to.
