What’s in a Name?
How you list your name is important for many reasons. We ask for your full legal name. We are finding that many names are not matching as far as IMPACT, IDFPR, and Provider Connections are concerned. If your IDFPR license uses a middle initial, so must your IMPACT. If you use a double last name, your license, IMPACT, and Provider Connections must all match. We have many people who send in a name change and only list the married name as the last name, not a double last name.
IDPFR automatically updates the license upon renewal with IMPACT. If your license uses a middle initial but you did not include a middle initial with IMPACT, the license will not update. IMPACT will note the name on the license does not match. This will interfere with associating with a different agency or becoming an individual provider.
Provider Connections cannot approve revalidations or update new associations until the name issue is addressed. This causes EI to not receive payment. Notices are being sent to providers as soon as this is noticed.
