November 17th is the deadline for submitting W-9s for certification to receive the incentive payments. Many providers have not completed the process. A provider may be currently inactive but had an active caseload and credential on June 30, 2023 and be eligible for the incentive.

If you do not submit a W-9 with your SSN, we will not be able to process an incentive payment for you. Please let any providers you know who may not be aware of the incentives know about this process.
