For providers eligible to receive incentive payments, it is vital that Provider Connection has your current home address on record. In some instances, incentive payments will be sent to provider home addresses and checks will not be forwarded by the US Postal Service. Provider Connections collects home addresses on the new credential application and on subsequent renewal applications. We ask all providers that if your home address has changed since you applied for or renewed your credential, please forward the new home address to providerconnections@wiu.edu before September 20, 2023.
